Complete the following form and click the Submit button to register for the 1st Annual FWU Golf Tournament. Individual players (i.e. not registering on a team) should complete the Sponsor/Golfer #1 Name section only. Teams can submit all team players (up to 4) with a single registration.

Submitting your application online does not guarantee acceptance. You must also mail your roster and application fee to complete the tournament registration process.

Fort Worth United Soccer Club is a non-profit organization dedicated to the ongoing development of soccer skills for both young men and young women. To reach that ultimate goal, FWU provides year round skills sessions conducted by our nationally licensed coaches and trainers under the direction of Mike King (Director of Club Operations).
